FOR NEW STUDENTS
- Report Card (Form 138-A)
- Certificate of Good Moral
- PSA Certificate of Live Birth (2pcs Photocopy)
- 2x2 Formal ID Picture with Name Tag and White background
- Long Brown Kraft Envelope
FOR TRANSFEREES
- TR for evaluation purposes
- Certificate of Transfer Credentials (Honorable Dismissal)
- Certificate of Good Moral
- PSA Certificate of Live Birth (2pcs Photocopy)
- 2x2 Formal ID Picture with name tag and white background
- Long Brown Kraft Envelope
GRADUATE SCHOOL
Enrollment Steps
FOR NEW STUDENTS
- Visit the Dean's Office
- Interview with the Dean
- Secure initial advice and clearance for enrollment
- Proceed to Registrar's Office
- Submission of Requirements
- Get your Enrollment Form from the Registrar
- Go to the Cashier's Office
- Pay for your enrollment
- Keep the official receipt (OR) as proof of payment
- Return to the Registrar's Office
- Submit 1 copy of your filled-out Enrollment Form signed by the Cashier
- Submit to the Dean's Office
- Write the following at the back of the Dean's copy:
- Facebook Name
- Contact Number
- Description of your Facebook profile picture (Example: Wearing white shirt with black background, or with graduation cap)
- Submit the Dean's copy of your Enrollment Form
- Write the following at the back of the Dean's copy:

Do not set your Messenger or Facebook account to private, and keep your message request settings ON so you can receive official messages and updates.
For inquiries regarding the total amount of fees for the whole semester, please proceed to the Cashier's Office for proper assessment and clarification.
